For a 15 year old female this should be a physical, the nurse will check the basic stuff like blood pressure and height and weight and stuff. Then they make you change into this slip. The doctor does the typical physical including listening to your heart, feeling your throat, looking in your ears and mouth and of course check your breasts and look down there very quickly to check for pubic hair growth, etc.
They ask you questions like are you sexually active, when was your last period, have you every drank alcohol etc.
It's nothing to be scared of.
It's very different from when you are a little girl though.
- Edit I just realized you were a guy and if you are a father just so you know, I don't know if your daughter hasn't had a physical since she was a young girl, but you don't go in the physical with her anymore. [Usually this stops at around 11] All the questions they ask her are confidential like if she has had sex or not.What happens in a teenage girls physical?
They listen to her heartbeat, check pulse, take height and weight, test reflexes, ask health related questions, discuss any medicines she's on or taking regularly. And answers any questions the parents or the girl may have.
There will probably not be a breast and pelvic exam at this point- but some girls may have one depending on family history of cancers and the doctor.

The doctor checks your balance, then reflexes, then heart, ears, eyes. After thats finished the doctor will tell you to get into that ugly slip thingy so he/she can examine you.....
